     Notice that the pitcher, catcher and 1st have decisions to make.  They make these decisions in unison as a team at the crack of the bat.  Communication is key.  The alternative mentioned in the drawing would give more options for throwing quickly to second or third.

By Position:
Pitcher decides to either back up the cutoff by positioning himself at the SS vacancy, or moves to backup 3rd.  
Catcher moves to follow the runner to first if it looks like it will be a close play at second.  If the runner moves past second, you must get back to the plate quickly.  If you know it's going to be at least a double, you should just move out to protect the plate.
First Base stays near enough to the base if a slow runner might be forced back.  If you see a play at third or the plate is likely, position yourself to backup a throw or take a second cutoff.  Remember the cutoff stance. 
Second Base covers the bag to keep the runner from advancing.  You may need to move out to relay a quick cutoff home depending on where the ball is hit and how fast the runner is, but generally you stay at the bag.  Hopefully there will be a play.  Remember to sweep the tag.
Third Base tags the runner out coming to third.  Remember to get your glove on the ground in front of the bag and the sliding runner.  If you get the throw and the runner retreats, be very selective on throwing to second.  Remember the infield's primary responsibility is to keep the runner at second.
Short Stop moves out to cutoff the throw.  When you're in position or on your way, check the runner and be ready to throw it to 2nd or 3rd.  Your teammates will help by calling to you, but it helps to know yourself.
Left Field runs quickly to the ball.  Communicate with the CF.  Either get the ball and quickly throw it to the cutoff, or back up the CF.  If you become the backup here, try to see or hear where the throw should be made, and tell the CF.  
Center Field runs quickly to the ball.  Communicate with the LF.  Either get the ball and quickly throw it to the cutoff, or back up the LF.  If you become the backup here, try to see or hear where the throw should be made, and tell the LF.
Right Field moves in to backup a throw to second.  The throw could come from either the outfield or back from 3rd.  Be ready for either.  Watch the play.  You are an important link in communication.  You often have the best view of the runner in relation to the ball.  Yell this information to your teammates.  It would be a shame to keep it a secret.
